چکیده

We read with interest the report by MONTANI et al. [1] of fatal rupture of pulmonary arteriovenous malformation (AVM) in a patient with hereditary haemorrhagic telangiectasis (HHT) and severe pulmonary arterial hypertension (PAH) confirmed by right heart catheterisation. The authors hypothesised that bleeding from pulmonary AVM may have been facilitated by increased mean pulmonary artery pressure (Ppa). Here, we report a patient with near-fatal haemorrhage from pulmonary AVM, probably facilitated by increased cardiac output, but without severe PAH.

Recurrent Haemorrhage Due to Cerebral Arterio-Venous Malformation in Successive Pregnancies: a Rare Presentation

Introduction: The rupture of an intracranial Arteriovenous Malformation (AVM) in pregnancy is a rare occurrence, but may have fatal maternal and fetal consequences. Association between AVM rupture and pregnancy has been proposed which may be caused by increased cardiac output or circulatory effects of the elevated estrogen levels. The presentation during the pregnancy is usually as a result of ...

Direct haemodynamic effects of pulmonary arteriovenous malformation embolisation

BACKGROUND Transcatheter embolisation is widely used to close pulmonary arteriovenous malformations (PAVMs) in patients with hereditary haemorrhagic telangiectasia (HHT). Data on the direct cardiovascular haemodynamic changes induced by this treatment are scarce. OBJECTIVES We investigated the direct haemodynamic effects of transcatheter embolisation of PAVMs, using non-invasive finger pressu...

Cerebrovascular manifestations in 321 cases of hereditary hemorrhagic telangiectasia.

BACKGROUND AND PURPOSE Patients with hereditary hemorrhagic telangiectasia (HHT) are at risk for developing cerebral vascular malformations and pulmonary arteriovenous fistulae. We assessed the risk of neurological dysfunction from these malformations and fistulae. METHODS Three hundred twenty-one consecutive patients with HHT seen at a single institution over a 20-year period were studied. A...

A pulmonary right-to-left shunt in patients with hereditary hemorrhagic telangiectasia is associated with an increased prevalence of migraine.

INTRODUCTION Hereditary hemorrhagic telangiectasia (HHT) is a rare autosomal-dominant vascular dysplasia with a high prevalence of pulmonary arteriovenous malformation (PAVM). Recent studies report an increased prevalence of migraine in patients with a cardiac right-to-left shunt. The aim of our study was to evaluate whether there is also an increased prevalence of migraine in patients with a p...
